Welcome to DMC Express Check-In! With our new Pre-Check and Check-In options, you will be able to save time and avoid waiting in line when you come in for your appointments.
Prior to your appointment, you will receive a visit confirmation via text and email. Please choose one to confirm your appointment and you will receive Pre-Check links to complete your visit paperwork ahead of time, at your convenience. Completing Pre-Check will save significant time when you arrive for your appointment.
When you arrive for your appointment, find the Check-In Kiosk or Check-In QR code and follow the prompts. You will be asked to scan or upload your license and insurance card. (Patients seeking Walk-In Care in Bedford or Londonderry please check in with a receptionist instead of using a kiosk or QR code.)
Once you have completed Check-In, you can relax and wait to be called for your appointment.
*Check-In Kiosks are located in the following DMC clinics: Bedford, Concord, Derry, Londonderry and Raymond. All DMC clinics offer QR code check-in.

FAQ’S
Q: Why is DMC installing kiosks and providing QR codes for Check-In?
We’ve implemented a digital registration process to eliminate paper, increase the accuracy of the information received and reduce your check-in time. The kiosk/QR also provides more privacy for you and better security around the information you give our practice. And, it allows our staff to spend more time with patients who need assistance.
Q: What is Pre-Check?
We’ve implemented a Pre-Check process where you can update and complete all your forms at home, reducing the time you spend at a kiosk, or mobile phone at DMC. The next time you get your email or text appointment reminder, click Pre-check after you confirm your appointment and you will be able to complete up to 90% of your visit paperwork ahead of time.
Q: Is my privacy protected?
Yes. We follow federal standards to protect your information. The system we use is HIPAA-compliant, and your information is fully encrypted, which allows us to protect your data even more securely than before.
Q: Why do I need to scan my driver’s license and insurance card every time I come in?
You’ll scan your driver’s license when you check in for an appointment so we can verify that it’s you and make sure we have the most up-to date insurance information.
Q: What if I don’t want to pay with a credit or debit card or have a question about the amount due?
You can select the Cash or Check button on the payment screen when you’re checking in and then pay by cash or check at the front desk. We can review your co-pay or balance with you at the front desk – just select the Cash or Check button on the payment screen when you’re checking in.
Q: Why was I asked multiple times to complete the same question? If you skip a question or do not hit the complete button during the pre-check process, you will be asked to complete the question or the information again. Some forms have to be completed every visit but most of the forms are programmed to be asked annually or once and never asked again.
