What Can You Do in the FollowMyHealth Patient Portal?

The FollowMyHealth (FMH) Patient Portal keeps all of your health information in one, easy-to-access location and allows you to communicate securely with your healthcare team, view your clinical summaries, refill your prescriptions, view/pay your medical bills and more.

When you log into FollowMyHealth (FMH) you will be connected to your personalized home page. On this page, you can see a snapshot of some of your health stats (like your height, weight, BMI, and blood pressure from your latest visit), view important health reminders, and navigate to other sections of the portal.

 

Please note that DMC has discontinued service with the Klara messaging platform. We encourage you to use FollowMyHealth instead to send secure messages to your healthcare team.

 

Please click on the tabs below to learn how to navigate to some of the more frequently accessed features of FMH.

FMH allows you to send messages to and receive messages from your healthcare team.

 

Using a Browser:

From the main menu, click on the Messages tab. You will immediately see any messages that have been sent to you from your healthcare team. Click on any message to open and view it. You can also create a message for your healthcare team by clicking on the “Compose” tab near the top of your screen. Follow the instructions to choose the provider and enter your Subject and Message before clicking “Send.”

Using the FMH Mobile App:

From the main menu, click on Messages. You will immediately see any messages that have been sent to you from your healthcare team. Click on any message to open and view it. You can also create a message for your healthcare team by clicking on the message bubble at the bottom of your screen. You will need to choose the provider and then enter a Subject and Message. Then press “Send.”

You can request refill/renewals on eligible medications through FMH. Please note that we require THREE BUSINESS DAYS to complete your refill request. If you cannot wait three business days, please call our refill department at 603-537-1300 and press extension 3.

Using a Browser:

From the main menu, click on the “My Health” tab at the top. Then click on “Medications.” You will see the pharmacy we have listed as your preferred pharmacy. You can change your preferred pharmacy by clicking the “Edit” button. You will also see a list of your medications. To refill a medication, simply click on the icon that looks like a medicine (Rx) container on the same line as that medication.

Using the FMH Mobile App:

From the main menu, click on Renew Prescription. Select the prescription you wish to renew from the drop-down choices. Review the pharmacy name and location listed and update if needed. Choose your provider by clicking on the drop-down menu under provider. Then click the arrow to continue to the next screen. If your prescription is eligible for a refill, you will receive a pop-up message confirmation that your request has been sent to your provider for a refill.

Your Clinical Summaries from your visits are available for your to view in FMH.

Using a Browser:

From the main menu, click on the “My Health” tab which will bring you to your health summary. You can also click on other important tabs from here, such as Conditions, Allergies, Immunizations, Vitals, and Documents.

Using the FMH Mobile App:

From the main menu, select My Health and click on Documents and on Clinical Summary Documents and select the Summary you wish to view.

In FMH you can view your unpaid bills, bills that are pending insurance claims, and paid bills. When an invoice is added to your account, you’ll see it on your home tab as an action item and under billing on your homepage. Just click “Make A Payment” to pay your bill online.

Using a Browser:

On the main home page, you will see a section for “Billing” on the right. Here you will see any current invoices and you can click on “Details” to  learn more.

Using the FMH Mobile App:

From the main menu, click on Bills. From there, click on the tab you wish to view (Unpaid Bills, Pending Insurance, Paid Bills). Click on Show Details to view the details of an individual bill.

FMH makes it easy to see upcoming and past appointments.

Using a Browser:

On the main home page, you will see a section for “Appointments” on the right. Clicking on the Upcoming tab will show you the date, time, provider, and location of any upcoming scheduled appointments. If you click on an individual appointment, it will show more detailssuch as the type of appointment that is scheduled as well as buttons you may click on to Cancel or Reschedule the appointment. Clicking on the Past tab will show the same details for past appointments. The Other tab shows appointments you have cancelled.

Using the FMH Mobile App:

From the main menu, click on Appointments. Then simply click on the tab you wish to view. The Upcoming tab will show you the date, time, provider, and location of any upcoming scheduled appointments. You can access your lab results from bloodwork ordered or performed by DMC.

From the main home page, click on the arrow next to the My Health tab at the top and then click on Results. You will see results for labs we have in our system for you.

Using the FMH Mobile App:

From the main menu, select My Health and then click on Results. You will see results for labs we have in our system for you.

You can view your imaging reports in FollowMyHealth.

Using a Browser:

From the main page, click on the arrow next to the My Health tab at the top and select Documents. Choose the report you want to view

by clicking on it (e.g. Mammogram, CT Scan, MRI, etc.)

 

Using the FMH Mobile App:

From the main menu, select My Health and then Documents. Choose the report you want to view by clicking on it (e.g. Mammogram, CT Scan, MRI, etc.).

How Do I Sign Up for the FMH Patient Portal?

New users can sign up for the FMH Patient Portal by CLICKING HERE or by clicking on the blue button that says, “FMH Patient Portal” at the top our homepage (DMCPrimaryCare.com). Click “I Need to Sign Up” and then follow the on-screen prompts. Be sure to connect DMC to your account once it has been created.

Another way to sign up is through an email you may have received when you established care with DMC. When a new patient provides DMC with their email address during registration, they are automatically sent an invitation to create a FMH account via email. You can also sign up using that invite email.

  1. Check the email account for the email address you provided during DMC new patient registration.
  2. Click the registration link in the email you receive from noreply@followmyhealth.com.
  3. Create your portal login using a unique username and password.
  4. Enter the invitation code provided by us. Then, follow the on-screen prompts to complete your account.

If you need a new invitation to be sent to you, simply call our Patient Service Center at 603-537-1300 and let them know you’d like a new FollowMyHealth Patient Portal invitation. They will confirm your email address and have aa new invite sent. Then, just follow the steps above.

How Do I Sign Up My Child for the FMH Patient Portal?

Parents and Guardians can set up a FMH Patient Portal account for minors under the age of 12. If you wish to set up a portal account for a minor in your care, please call 603-537-1300 and enter extension 2517. We will guide you through the process of setting up account(s) for your child(ren).
